RE-SCHEDULING
For changes regarding appointments, patients should inform the website (via email, Viber or WhatsApp) at least 24 HOURS before their original schedule. The new consultation schedule will be subject to availability of the concerned physician.
Patients are encouraged to be prompt and follow their consultation schedule. A maximum allowance of 15 minutes will be given to the patient to appear for the online session. Otherwise, the consultation will be forfeited and considered as a completed service.
CANCELLATIONS
Cancellation will result in the forfeiture of your payment except for certain emergencies (ex. sickness, death, etc.)
I understand that my doctor can discontinue the telemedicine consultation if it is felt that the chat/video conference connections are not adequate for the situation.
I understand that there is no guarantee that this Telemedicine Consultation will eliminate the need for me to see a doctor in person. Accordingly, I will not hold my doctor liable for any incomplete or misdiagnosis if the same was a result of my having withheld any material information or due to inherent limitations of this telemedicine consultation.
I understand that the physician and medical staff involved in this teleconsultation are entitled to a reasonable compensation which shall be settled in the most expeditious and judicious manner.
Tele-consult cannot fully replace face to face consult.
Doctor will not be liable for any untoward course your condition or any security matters that may arise.
